Linkage will primarily happen through your case management process.
We can help with:
- Connecting you to outpatient behavioral health services with a psychiatrist and/or therapist
- Connecting you to community resources that can help with your transportation, housing, financial, medical, or other identified needs
- Ensuring that your Family and Friends are linked with caregiver support resources
To ensure you experience Continuity of Care, we will coordinate your treatment planning with:
- Outpatient Behavioral Health Providers such as your Psychiatrist, Therapist, or Medical Provider.
- Your Identified Support Systems. These may include Family, Friends, or Community Resources
